This session is FREE to members of the Special Education Cooperative.

Effective transition planning within the Individualized Education Program (IEP) is critical for preparing students with disabilities for post-secondary success. This session will explore best practices in IEP transition planning, emphasizing collaboration among the other silos of education including CTE, CCMR, PGP with counseling and community agencies. 

Participants will gain insights into legal requirements, person-centered planning, and strategies for aligning transition goals with students' strengths and aspirations.
