Using Present Levels of Academic Achievement and Functional Performance (PLAAFP) to Create Effective Post-Secondary Transition Plans for Students with Disabilities



ESC-20 provides training and technical support related to inclusive services for students with disabilities to gain access to the general curriculum. These supports provide strategies so students with disabilities are actively involved and progress within the general curriculum and on state assessments.

Important Session Information:


This session is FREE to members of the Special Education Cooperative.

Target Audience:
This session is designed for:

  • Special Education Teachers
  • General Education Teachers
  • School Administrators
  • Transition Employment Designees (TEDs)

Pre-Session Recommendation:
Participants are strongly encouraged to complete the free, asynchronous Standards-Based Individualized Education Program (SBIEP) Process Training prior to attending this session.

Session Overview:
This interactive training will guide participants in using PLAAFP components to develop meaningful and compliant post-secondary transition plans for students with disabilities. Emphasis will be placed on applying PLAAFP data to inform transition services and engage students, families, and teams in the planning process.

Morning Focus:
Participants will:

  • Practice applying concepts from the SBIEP training
  • Receive coaching and feedback
  • Engage in hands-on activities targeting students preparing for post-secondary outcomes

Afternoon Focus:
Participants will:

  • Connect PLAAFP statements to post-secondary transition plans and IEP goals
  • Examine legal requirements and best practices
  • Use case studies and group collaboration to develop a sample transition plan based on a provided PLAAFP

What to Bring:

  • All materials provided during both days of the SBIEP training
  • A current sample PLAAFP and associated IEP goal(s) for at least one student on your caseload
