Transition planning isn’t just for high school, it’s a journey that begins long before students reach graduation. This session explores how educators and families can intentionally build the skills, confidence, and self-advocacy of our youngest learners so they are prepared to actively participate in their futures.
Participants will discover practical ways to embed self-determination, communication, choice-making, problem-solving, and independence into everyday learning experiences. We’ll explore how early experiences and expectations can lay the foundation for successful transitions across grade levels and, ultimately, into adult life.
The session will also focus on strengthening partnerships between educators and families by moving beyond short-term, school-year planning and creating a shared vision for a child’s long-term development. Participants will explore effective planning tools, strategies, and resources that help families and school teams identify strengths, interests, needs, and future possibilities while ensuring the student remains at the center of the process.
Come ready to rethink what transition planning can look like in early childhood and elementary settings—and leave with practical strategies to help students begin finding their voice, making choices, and building a vision for their future from the very beginning.
