This session will a cover an overview of the transition process to include:
- Transition assessments;
- Annual individual education program (IEP) goals and post-secondary goals;
- Present levels of academic achievement and functional performance (PLAAFPs);
- Course of study;
- Coordinated set of activities;
- Self-determination;
- Summary of Performance; and
- Agency Linkages.
New Transition and Employment Services Designees (TEDs) and teachers involved in the transition process are encouraged to attend.
